Job Title: Case Manager
Program: Refugee Match Grant
Job Summary: The purpose of Refugee Match Grant is to provide both employment and case management services to refugee singles, couples, and family units. In the provision of employment services, the Match Grant Case manager conducts comprehensive intakes and assessments of client needs and job skills; develops relationships with employers in the community; and assists the client with American work culture orientation, assists the client in acquiring the necessary soft skilss to effectively participate in job search, and connects clients with job opportunities leading the client towards self-sufficiency.
Job Requirements:
1. Interviews clients through comprehensive intake and assessments. CM assesses to determine the barriers to employment and to discover the clients' job skills, as well as other case management needs such as personal and family adjustment, finances, food, clothing, housing, and health.
2. Develops, implements, monitors, evaluates, and updates individualized service plans according to the guidelines set by the programs and to review progression towards goals with the client.
3. Network with employers, build positive relationships within the business community, and provide cultural orientation to employers about refugee populations.
4. Helps client to identify, establish relationships with, and/or expand existing social network, social service providers, churches, and refugee community representatives to enhands household functioning and integration into the community.
5. Empouwers client towards cultural acquisition and comprehension of the new community through an understanding of self, personal situation, and client's responsibility for adjusting to new life in local community.
6. Works with Match Grant Community resource Developer to fulfill volunteer and in-kind needs of the match Grant Program.
